In the attached example, there is a separate page that gives alternate directions for students who are working from home and do not have a device that easily allows them to use the drawing tools. If your students are working in the classroom and you do not need the alternate directions, delete the second page in the Attached Example and the third page in the Template before sharing the activity with your students. Before this assessment, students should have had lots of experiences with these letters and letter sounds. You can see other activities on my author's page that lead up to this assessment. The same pictures used in this assessment have also been using the previous activities so students should be familiar with them. Depending on your students' experience with name writing, students may use all uppercase letters or a mix of uppercase and lowercase letters to write their names. The example uses title case, where only the first letter of the name is uppercase.
